This form is to be completed by students who are enrolling in an Associate Degree program or have reached the designated checkpoints for the HOPE scholarship review. These checkpoints are 45, 90, and 135 attempted hours. Please complete this form and submit it to the Registrar’s office.
This form is to be completed if you are interested in applying for a Federal Work-Study position. Student must be Pell eligible and enrolled in a Pell eligible program to participate.
